- Your date of birth
- The email address we have on file for you
Your details stay private. We email you a one-time code before anything is shown, and the code is sent only if the address you enter matches the one on your file.
One record across U Vision Group
U Eye Laser Cosmetic shares a single clinical record with U Dry Eye Institute, Uptown Eye Specialists and the other U Vision Group clinics. Any upcoming appointment you have with them shows here too, with that clinic’s name and address.

If your consultation is virtual, it is marked as virtual and no address is shown.
Need to change or cancel a visit? Call us at (416) 292-0339 or email info@uelc.ca. This tool shows your appointments but cannot change them.
