This website is operated by UELC Inc., which carries on business as U Eye Laser Cosmetic ("we", "us"). By using it, you agree to these terms. If you do not agree, please do not use the site.

1. This website does not provide medical advice

We make this website available to provide useful information to the public, not to provide medical advice or health care services. Nothing on it is a diagnosis, a treatment recommendation, or a substitute for speaking to a qualified eye care provider about your own eyes. Please consult your physician or optometrist about your specific diagnosis, treatment and health questions, and never delay seeking care because of something you have read here.

This website is not monitored and is not for emergencies. If you need urgent medical attention, call 911 or go to your nearest emergency department. For an urgent concern about your eyes, call us at (416) 292-0339.

2. Appointment lookup

Appointments with U Eye Laser Cosmetic can be viewed through our shared appointment lookup, hosted at uptowneye.ca — U Vision Group's clinics share one clinical record. It displays appointment information from that record. It does not show every appointment, and it cannot be used to book, change or cancel one — call the clinic shown on the appointment to do that. If you expect an appointment and do not see it, please call us rather than assume it is not booked.

You may use it only for your own appointments, or for someone whose information you are lawfully entitled to access. Access is logged.

Verification codes are sent only to the phone number or email address already on your patient file and are for your use alone. We will never telephone you and ask you to read a code back to us.

3. Website content and availability

We take care to keep this website accurate and available, but we do not warrant that it will be uninterrupted, error-free or current. Information can become out of date; our clinical record, not this website, is the authoritative source for your care. We may change or withdraw any part of the site at any time.

4. Intellectual property

The content of this website, including text, images, logos and brand names, belongs to UELC Inc. or its licensors. You may view and print pages for your own personal, non-commercial use. Any other reproduction or redistribution requires our written permission.

5. Links to other websites

Where we link to other websites we do so for convenience. We do not control them and are not responsible for their content or their privacy practices.

6. Privacy

Our handling of personal and personal health information is described in our Privacy Policy, which forms part of these terms. We handle personal health information in accordance with Ontario's Personal Health Information Protection Act, 2004 (PHIPA).

7. Limitation of liability

Your use of this website is at your own risk. To the fullest extent permitted by law, UELC Inc., its physicians and its staff are not liable for any direct, indirect, incidental, special, consequential or punitive damages, or for any loss of data, arising from your use of — or inability to use — this website, including any reliance on information it contains.

Nothing in these terms affects, limits or replaces the professional duty of care owed to you in the provision of health care. Nothing in these terms limits or excludes any liability that cannot be limited or excluded by law. Some jurisdictions do not permit the exclusion of certain damages, in which case the paragraph above may not apply to you.

8. Governing law

These terms, and your use of this website, are governed by the laws of the Province of Ontario and the applicable laws of Canada. You agree that the courts of Ontario have exclusive jurisdiction over any dispute arising from them.

9. Severability

If any provision of these terms is found to be invalid or unenforceable, that provision will be severed and the remaining provisions will continue in full force and effect.

10. Changes to these terms

We may update these terms from time to time. The date above shows when they were last changed, and the current version applies.
